1- What is your real name?[you dont need to answer]: I would Like to remain Unknown.
2- How old are you?: 14 Years ;D
3- Which time zone are you in? (Ex., EST/ PST): GMT
4- How many hours a day can you play? Per Weekend?: 3 - 12 Depending on the season.
5- Have you ever been a staff on any other Private servers (proof would be nice)?: Many

7- Do you have a good knowledge of staff commands? I've picked up a few while ingame, I could learn them In minutes.
8- What do YOU think is the most important duty of being a Mod?: Helping others and being active.
9- What is your character name on our server?: Darkness
10- What makes you think you are qualified to be a Mod on ForgivenPk?: I help whenever I'm able to, I treat players and members of staff with respect, If they treat me badly, They will be treated badly back, Treat how you'd like to be treated.
11- Any other relevant information?: Not that I know of.
12- Did you take your time on this?: I took around 10Minutes of courage to write this, Yes.
13-Why should you get Mod?: I've mentioned this answer in Q-10
14- How often will you contribute to forums?: Daily, I always have a Forum Tab open While InGame.
15- Would you consider yourself a good moderator?: I would consider myself a Fun but Deadly moderator, Meaning It's all fun and games until you break the rules, I will enforce them if you're my enemy, My family or my friend.
